The Client Contact Form gathers general information that is vital for us to open accounts and manage your money within our systems. This form generally takes about 15 minutes to complete.

2) Please fill out the Financial Factfinder Form.
Our Factfinder Form is designed to provide us with a detailed understanding of your financial status. This survey allows you to link your existing financial accounts, including checking accounts, employer retirement accounts, credit cards, student loans, and much more, provided there is an online account with the institution. In addition, the survey includes a secure, encrypted Vault Storage where you can upload necessary documents before your appointment.
Before taking the survey, please review the pre-appointment checklist to ensure you have all the recommended documents.
If you don’t have everything, don’t worry; submit what you can. Our aim is to get a complete picture of your income and debt, so we can assist you in achieving your financial goals.
This survey generally takes about 30- 45 minutes to complete.
